Kozel DDS, Thomas age 55, beloved husband of Priscilla "Prill" (nee Field), devoted son of Robert W. and Jane Kozel of Northfield, loving brother of Mary (George) Liebrock and Peter (Mary Jo) Kozel, loyal brother-in-law of Daniel (Kathy) Field and Michael (Lorraine) Field, fond uncle of Kevin and Kelly Liebrock, PJ and Tom Kozel, John and Sara Field and Emily, Andrew and Katherine Field. Dedicated partner in Albany Park Dental Center, 3403 W. Lawrence Ave. Graduated from the University of Illinois at Chicago and the University of Illnois Dental School, where he taught operative dentistry from 1979-1990. Passed away May 12, 2005. A Memorial service will be held Wednesday, May 18, 11 a.m., at Church of Our Savior, 530 W. Fullerton Parkway, Chicago, IL, 60614 (www.coschurch.org). In lieu of flowers, donations for colon cancer research to the memorial fund of Thomas F. Kozel at Northwestern Unversity, Office of Development, 710 N. I was "Tommy" Kozel's dental patient when he was a student at U of I Dental School--quite a few years ago. He used to refer to me as his little "goldmine." He was able to make all his gold requirements with my mouth--even a gold foil (which he needed to do, so he paid the extra dollar for the foil). Not only did he lend me out for another student's gold requirements, I was his "gold" board patient. After he graduated, I ended up going to his teacher, Ronald Giometti (we used his office for the board x-rays). I think I still have a few of his onlays left in my mouth after all these years. Please accept my sincere condolences. I truly enjoyed knowing Tom and I have been fortunate to have lived with beautiful, quality dental work from his student days.

I went to UIC with Tom for four years. We were good friends and he was always a great and fun-loving guy. Unfortunately, after graduation, I went to Medical School and Tom to Dental School and we lost track of one another, but I always had fond memories of our years in College.

I'm certain he was an outstanding practitioner, as he always did everything with utmost perfection.

A kind, gentle, and caring man, he will be missed.

I have been a devoted patient of Dr. Kozel since he started his practice with Dr. Jacobson. We used to have a great time joking around (believe it or not) as he cleaned my teeth or even did some work. If it hadn't been for his concerns and caring about the individual, I would have undergone many very painful procedures. I will miss him, he will very hard to replace. For many years we always went to see Dr. Kozel as a family on a day the kids were out of school or during the summer. He gave us such good care, was always so kind, and we will miss him terribly. May God bless his family. You are in our prayers.

John was a patient of Dr. Kozel for over 20 years, and will miss him greatly. I have only been a patient for about 15 years, and have not always been a very good patient, but Dr. Kozel never gave up on me. We will miss him - he was taken from us all too soon. Rest in Peace.

We both had the pleasure of first coming to Dr. Kozel about 20 years ago when he had his office in Uptown. My husband and I admit that we were not always the best patients but agree that he was the best dentist. He will be greatly missed.



How many people can say they cried of laughter while at the dentist. I can. I remember once Dr. Kozel and I were sharing stories about how we hated the damage squirrels were causing to our houses. As we each pushed forward with who had the worst story our conversation evolved into creative ways to eliminate those pesky critters. Some of the methods he was coming up with were totally hysterical and me with my mouth fill with instruments was crying with laughter. This side of Dr. Kozel made "going to the dentist" "not so bad".
