David Forkosh Obituary

Forkosh, MD, David S. Age 74; beloved husband and life companion of Linda Hirshman nee Redlick; very loving father of Margot Atlas and Elyse Forkosh (David) Cutler and adoring stepfather of Sarah "Shari" (Seth) Shapiro; proud grandfather of Yair, Matan and Meital Atlas, Hannah and Samuel Cutler, Sylvie and Sydney Shapiro; dear brother of the late Ruth (the late Sheldon) Waldman; devoted son of the late Sydney and Bertha Forkosh. Dr. Forkosh was a respected physician, hospital executive and longtime president of the FMH Foundation. He will be dearly missed by his loving family, friends and colleagues. Chapel service Wednesday, 2:30 PM at Shalom Memorial Funeral Home, 1700 W. Rand Road, Arlington Heights. Interment Shalom Memorial Park. Dear Linda,

I just today - May 17th, learned of David's death. Though we were cousins and far from close, I have fond memories of him at my father's funeral (1961) and talking to you when my daugher Melanie was diagnosed with Hodgekins in Arizona in 1994 from which she made a complete recovery and is now a mommy to 2 little ones. Since Jerry Forkosh passed away in December 2007, Forkosh Family News stopped.

Dear Linda and all the Family,
We mourn with you the death of a man who had become a dear friend with whom we shared many delightful times--in both New York and AZ. Those are treasured memories, and we feel honored to have been able to share David's last High Holidays with the two of you.

Dear Linda and Family,
It was with great sadness that I read of David's passing. We were very close for a good many years, High School through Medical School and beyond. Our fathers were intern classmates at Cook County Hospital in the 1920s, a fact uncovered when we met in a Biology class at Roosevelt High School. We spent many wonderful times together; I also got to know his mother Bertha and sister Ruth well in those years. While David's and my career paths and our geographic locals diverged significantly, I have most fond memories of those close years and now regret having not kept in better contact over the past decades. David had an influence on me in many ways for which I have always been grateful. I will always remember him warmly. He will be missed.

David will be missed. He was a great guy and one of my good clients. Simply, we we trusted each other and enjoyed working together. Articulate, great sense of humor and obviously a good taste in people.

Dear members of the extended Forkosh family,
It is unlikely that any of you other than Linda will recognize my name. I am a cousin to David through his Mother Bertha who was related to my mother Rena Waller Karchmer. Linda we met once, likely more that a decade ago, when you and David were in Massachusetts at the Cape as I recall. David's death was called to my attention by a friend in Chicago who was a fellow camper at Camp Indianola. It is sad that families in America become so attentuanted. In spite of the distances in time and space, I was sorry to learn the news and send my condolences to each of you with the hope that fond memories will soften your loss over time.

I have a great memory of shooting with David at Lincoln Park and his company was fun in the Arizona desert on a long ago dove hunting trip with several other Lincoln Park members.
He will be missed!
