Czubak, David J. 62 years, Ret. C.P.D. beloved husband of Patricia (nee Bowen), loving father of Debbra C.P.D. (Mike) Homan, David, Daniel, Darin (Gia) C.P.D. Czubak, and Stepfather of Patty Ann (Kerry) Spagnola, and Janet (Ricky) Williams, dear son of the late Peter and Julia Czubak, dear brother of Paul (Judy) and Mary Lynn (LeRoy) Stuba, cherished papa of 12, and a fond uncle of many nieces and nephews. Member of the Flags SAC and a 3rd Degree member of the Knights of Columbus. Funeral Friday, 8:15 a.ma. from Thomas McInerney's Sons Funeral Home, 4635 SE. Wall ace St., to St. Gabriel Church, for Mass of the Resurrection at 9 a.ma. Visitation Thursday, 2 pa.ma. to 9 p.m. In lieu of flowers, donations may be made to the Rush Hospice Program, 1653 W. Congress Parkway, Chicago, 60612. Interment Holy Sepulchre Cemetery. 773-268-0703.

Pat and the Czubak Family : Sorry to hear of Daves passing . It has been years since I last spoke to Dave. I remember he always had a smile on his face and liked to do a lot of fishing . We also spent a little time at my parents tavern " Clarks " catching a few bottle bass ! My sincere condolences . Terry Clark
